3/1/10: Quarterly meeting fosters continued partnership in care transitions
Emerson Hospital recently hosted a meeting, part of a larger initiative to improve care transitions, for community partners representing skilled nursing facilities, care management, and Councils on Aging, among other organizations. The goal of the meeting, titled “Care Transitions – Building Collaborative Partners,” was to develop standards for care transitions as patients transfer from the hospital to home or to further rehabilitation. Hosted by Emerson’s care management team, the meeting covered topics like advance directives, health care proxy, readmissions, and health care-associated infections, and allowed the team to introduce itself in a continued effort to strengthen partnerships in the community. “This was truly a community engagement effort geared toward improving the quality of care for our patients throughout the continuum of care,” said Dr. Sam Bagchi, associate vice president of utilization management and director of the hospital medicine service at Emerson Hospital.
