5/11/10: Emerson partners with Carleton-Willard to support area seniors
Emerson Hospital recently entered into an agreement with Carleton-Willard At Home to provide support to seniors living in Concord and several surrounding towns. The two organizations will be working together to enable residents to live safely, comfortably, and conveniently in their own homes. At Home, a not-for-profit membership organization and a sister organization to Carleton-Willard Village, was established last year to help seniors stay in their own homes with confidence and independence, while saving them money on a wide range of services from home maintenance to in-home health care. Available to residents of Concord, Carlisle, Bedford, Lexington and Lincoln who are over 65, At Home is one-stop-shopping, connecting members with carefully screened providers as well as providing fitness and social opportunities. And for those with transportation needs, At Home helps with the driving as well.
Under the new relationship, At Home members will have access to a designated social worker who will serve as a liaison with the hospital, available to address member issues that may arise, answer questions regarding various aspects of aging, and facilitate appointments. At Home members will also be able to enjoy discounts at the hospital cafeteria, parking and gift shop, receive members-only health talks and screenings as well as other benefits. Emerson Hospital has also provided At Home with a $500 grant to subsidize transportation to and from medical appointments.
