Getting Started
If your insurance requires it, you should contact your primary care physician for a referral to the program; this program is almost always covered by health insurance. Then, contact the Emerson Hospital Diabetes Education Program at 978-287-3054 to make an appointment.
At your first visit, a diabetes nurse educator will assist you in creating your diabetes self-care plan. Follow-up visits will then be scheduled depending on your needs.
The Diabetes Education Program will serve as a resource for you, and will assist you with questions, concerns, or problems you may have in regard to diabetes. It will also help you gain control of your disease, and help you lead a healthier life.
