Managing Your Diabetes
Together with your health care team, including your primary care physician, diabetes nurse educators, an endocrinologist (diabetes specialist), and a dietician, you will create a healthy plan to take charge of your disease.
Your plan is individualized and may include information on:
- Healthy eating and creating well-balanced meals.
- Regular physical activity.
- Checking blood sugar levels.
- Medication management.
Through the Emerson Hospital Diabetes Education Program, you may:
- Learn how to take responsibility for your day-to-day care.
- Learn how to keep your blood glucose levels from becoming too low or too high.
- Develop a schedule so you know when to see an ophthalmologist (eye doctor), podiatrist (foot doctor), a primary care physician, and other relevant health care providers. Seeing these professionals regularly, and when a problem arises, can help you avoid long-term problems.
- Explore options for using an insulin pump.
- Learn how to manage your diabetes during pregnancy.
