The Patient Experience

The goal of the Rehabilitation and Transitional Care Unit (TCU) is to help patients return home as soon as possible, while functioning as independently as possible. Patients receive the same high-quality, patient-centered care for which Emerson is known. Each patient is treated with dignity and respect and is encouraged to be involved in decisions about his or her care.

On a typical day, TCU patients:

Rehab care occurs throughout the TCU—in the well-equipped, sun-filled gym, common areas, kitchen/activities room, and residential bathroom.

Upon discharge, patients may take advantage of Emerson’s continuum of care, which can include Emerson Home Care or our Center for Rehabilitative and Sports Therapies.

Discharge Planning

Beginning on the day of admission, our care team considers all the rehab/nursing care and education required for patients to return home safely. The TCU social workers ensure that all the patient’s discharge needs are met when arranging for home care services. The interdisciplinary staff helps patients and family members learn the necessary skills to ensure a safe discharge home.

A Comfortable Environment, a Soothing Setting

The TCU is a 20-bed unit with semi private and four private rooms. It is a relaxed setting that features views of the Sudbury River and surrounding woodlands. A sitting room is available for visits with family and friends. The unit also features complimentary phones, free flat-screen TVs, and Wi-Fi for patients, family, and friends.

Appealing, nutritious meals are served in the attractively furnished dining room, which has a restaurant-style menu. The TCU’s dedicated dietitian can assist patients who have special dietary needs.

The Activity Director provides a variety of social and individual activities.

Patients can receive free hand massage and reiki treatments; beautician services and massage therapy are available for a fee. If needed, dental, podiatry, and ophthalmology services can be arranged. Certified pet therapy is also provided to patients who enjoy a visit from our trained "therapists."
