For your safety, due to the COVID-19 pandemic, we are currently offering telehealth rehabilitation services for both new and existing physical, occupational and speech/language therapy patients. These telehealth services are provided on the Doxy.me video platform and are offered Monday thru Friday, allowing you to seek the care you need from the comfort and safety of your own home and at a time that is convenient for you. Telehealth rehabilitation services are currently being reimbursed by most major insurance carriers. Please call 978-287-8200 or email EmersonRehab@emersonhosp.org with questions or if you are interested in scheduling an appointment with one of our therapists.
The following forms are intended for telehealth patients of the Clough Family Center for Rehabilitative and Sports Therapies. Patients are asked to fill out the appropriate forms and email them back to EmersonRehab@emersonhosp.org. These forms will open as PDF documents and the fields should be editable.
For all patients:
For pediatric patients:

We request 24-hour notification for cancellations. After three no-shows (no notification of cancellation) and/or cancelled appointments, the remaining appointments may be cancelled. Timeliness for your appointment is very important for the progression of your care; late arrival for appointments will be at the discretion of the treating therapist.
Tips to make your telehealth visit go smoothly
We recommend ensuring your device will work properly before the visit. Phones, tablets and computers with cameras can be utilized.
Please click on the appropriate Doxy.me link in the below chart before your visit to ensure that you can access the site and that your camera and microphone work properly.
Please wear comfortable clothing for your visit. It is helpful if you can move your camera around so that your therapist can possibly see you stand for postural assessment, walk for gait assessment, and lie down on your sofa, bed or floor to perform visual measurements as appropriate.
