How to Refer

​When you or a loved one needs home health care services, we make obtaining it as simple and easy as possible.

Patients may be referred for home care from virtually any health care setting, including an acute care hospital, a rehabilitation facility, a skilled nursing facility, or directly from a physician’s office.  

While a physician referral is necessary to receive home health care services, anyone — including the patient, a family member or professional caregiver — may request an initial evaluation from Emerson Home Care. If home care is determined to be appropriate, then one of our staff will contact your physician and request the required medical orders.

In most cases, home care services can begin within 24 to 48 hours of the referral.

It is that simple.
