Your baby’s birth certificate is an important legal document. Emerson hospital is required to complete and send the birth record to the Concord town clerk’s office within 10 days of birth. The birth record is completed by the birth registrar, who obtains the information from a state-issued parent worksheet and verifies the identification of the parents through valid government issued ID (driver’s license, state ID or passport).
